RYF will collect paperwork at the 2024 Season Kick-off on 06/20 at Coolidge Middle School fields. This paperwork is mandatory for your child to practice and play.
Items to Note:
- Medical Waiver: Either a physical dated after January 1, 2024 OR a signed medical waiver
- Waiver and Release of Liability Form
- Emergency Medical Treatment Consent & Information Form
- Code of Conduct Form (Player & Parent/Guardian)
- Image Release Form
- Report Card: Copy of student's final June 2024 report card
- Rising 8th Graders and all NEW players: Original birth certificate
- Returning AYF players: Do not need to provide a birth certificate
