RYF will collect paperwork. This paperwork is mandatory for your child to practice and play.
Items to Note:
- Medical Waiver: Either a physical dated after January 1, 2026 OR a signed medical waiver
- Waiver and Release of Liability Form
- Emergency Medical Treatment Consent & Information Form
- Code of Conduct Form (Player & Parent/Guardian)
- Image Release Form
- Report Card: Copy of student's final June 2026 report card
- FIRST YEAR players: Original birth certificate & a copy of the original (RYF will need copy certified by AYF)
- RETURNING players: Do not need to provide a birth certificate
